Privacy policy
Last updated:
At Lázaro Orthopedics we respect your privacy. This policy explains what information we collect through this website, why we use it and how we protect it.
Information we collect
When you complete the appointment request form, we collect: name, phone number, email, preferred office, whether you are a new or returning patient, area of concern, preferred days and time, contact preference and preferred language. We do not collect payment information through this site.
How we use it
We use this information only to contact you and schedule your appointment. We do not sell, rent or share your information with third parties for marketing purposes.
How we store it
Your request goes straight into the practice’s internal appointment system, where our staff manage it and contact you to confirm your appointment. The office receives an email notice, and you receive an email confirming we got your request (with no health details). Only authorized practice staff, each with their own username and password, can access the system. Information travels encrypted (HTTPS), and the providers that help us run the site, the appointment system and text messaging may use it only to provide that service to us.
Text messages (SMS)
If you call us and we can’t take your call, we may send you service text messages to help you schedule your appointment: first, a message that identifies the practice and asks whether you would like the link to request your appointment online and, only if you reply YES (SI), a second message with that link. We may also reply to messages you send us. Messages are sent from (787) 305-3299 and do not include health information.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. You can opt out at any time by replying STOP, and get help by replying HELP or calling (939) 585-8537. See our Terms of use for details.
WhatsApp and email
WhatsApp and email are not fully secure channels. If you write to us there, use them only to coordinate your appointment and don’t send detailed medical information, results or documents; we’ll discuss them by phone or in person.
The form is not for emergencies
The form is only for requesting appointments. Do not use it for emergencies — in an emergency, call 911 or go to the nearest emergency room. Please don’t include detailed medical history either; we’ll discuss it privately during your visit.
Cookies
This site does not use advertising or tracking cookies. It may only use essential cookies required for basic functionality.
Your health information (HIPAA)
As a healthcare provider, we protect your health information under HIPAA and Puerto Rico law. Information you share with the practice by phone, text message, WhatsApp, the form or in person is kept confidential and used only by authorized staff to coordinate and provide your care. How we use and protect your medical record, and your rights over it, are described in our Notice of Privacy Practices, which you can also request on paper at the office.
Your choices
You can ask us to correct or delete the information you sent through the site by emailing us. Requests that don’t lead to an appointment are kept only as long as needed to follow up. If you become a patient, your information becomes part of your medical record, which we must keep for as long as the law requires; you can then request a copy or a correction under our Notice of Privacy Practices.
